Before you tap “Submit” on the uFiling app or visit a Labour Centre, make sure every document below is on your phone or in your hand.
- Core documents (must-haves for every claim type)
- 13-digit SA ID – clear photo or certified copy
- UI-19 form – issued by your last employer (download from uFiling if they uploaded it)
- Retrenchment / dismissal / resignation letter – shows last day of work and reason for termination
- Bank confirmation letter or stamped statement (not older than 3 months) with your account number & branch code
- Latest IRP5 / IT3(a) – proves UIF contributions
- Extra papers for specific benefits Benefit Additional docs Illness Medical certificate from a registered practitioner Maternity Clinic card or doctor’s letter confirming expected due date, plus UI-2.7 form Adoption Court order or adoption placement letter Deceased contributor Death certificate, beneficiary ID, marriage or birth certificate proving relationship

- Lost UI-19? WhatsApp 060 012 3456 with your employer’s PAYE number; the bot e-mails a copy.
- No bank account? Open a Mzansi account at any big four bank—takes 10 minutes with your ID and proof of address.
- Missing IRP5? Download from SARS eFiling app → Tax Certificates → IRP5/IT3(a).
